INSURANCE & FEES

HEALTH INSURANCE PLANS

We are striving to get contracted with many insurance plans, but currently only accept: 

 

In-Network Plans (plans we accept): 

  • Aetna

ELIGIBLE STATES

Due to licensure laws in many states, we can only bill insurance for individuals that live in the states listed below.  If you do not live in one of these states or are an international patient, please see out of pocket fees and packages in the section below.   

  • Florida

  • California

  • Colorado

  • Washington

  • Idaho

  • Utah

  • Wyoming

  • Arizona

  • Oklahoma 

 

REFERRALS

If your insurance requires a referral, please be sure to send us the referral prior to the appointment.  The following insurance plans require a referral: 

  • All HMO plans

COVERAGE

If medical nutrition therapy is covered under your plan, we will bill your carrier and do our best to get your visit paid for by your insurance plan. If all or part of our services are not covered under your plan or apply to your deductible, you will receive a bill for the balance. 

 

In some cases, our services may be covered at 100% under preventative services. If you would like to call and verify your coverage prior to your visit, the link below shares some helpful tips and questions for you to ask to better understand your coverage.

*Please note that if you are a NAPA client or participating in an intensive feeding program with South Florida Pediatric Feeding Institute, these organizations handle the billing for their clients.  

FEES 

If you do not have insurance, are not eligible for insurance coverage or are an international patient, please see our 1:1 nutrition counseling options or our courses & programs.  1:1 nutrition counseling fees are listed below.  We are also able to provide you with a superbill so that you can submit to your insurance company for potential reimbursement.    

New Patient: $200

Follow up Visit: $150

Check In (15 min): $50

Lab Interpretation & review: $200
